Many people have heard the term gait assessment, but what does it actually mean, and how is it different from simply watching someone walk?
At its simplest, gait refers to the way a person walks or runs. However, a gait assessment is far more than simply observing movement. It is a structured clinical evaluation that helps identify how the feet, ankles, knees, hips and the rest of the body work together during walking or running.
Our walking pattern is influenced by many factors, including joint mobility, muscle strength, balance, previous injuries, footwear and even underlying medical conditions. Understanding how these factors interact can provide valuable information when investigating pain, injury or reduced mobility.
Why is gait assessment important?
The way we move affects how forces are distributed throughout the body. If movement patterns become altered, extra stress may be placed on particular joints, muscles or tendons, potentially contributing to pain or injury.
A gait assessment can help identify factors contributing to conditions such as:
- Heel pain (plantar fasciopathy)
- Achilles tendon problems
- Shin pain
- Knee pain
- Hip pain
- Recurrent sports injuries
- Balance problems
- Diabetic foot complications
It is important to remember that gait assessment is just one part of a comprehensive podiatry examination. It is considered alongside your symptoms, medical history and physical examination before any diagnosis or treatment recommendations are made.
Visual assessment or objective digital gait analysis?
Traditionally, clinicians have assessed gait by carefully observing patients as they walk or run. An experienced podiatrist can identify many important movement patterns through visual assessment alone.
Visual gait assessment relies on skilled clinical observation and interpretation. Objective digital gait analysis adds quantifiable measurements that can support and refine those clinical findings.
Modern digital gait analysis adds another level of detail. High-speed cameras and specialised software objectively measure movement, while pressure analysis systems can quantify the forces passing through the feet and how pressure is distributed during walking or running. Rather than relying solely on visual interpretation, these systems provide objective measurements that can be analysed, compared over time and interpreted alongside the clinical examination.
Unlike visual observation, objective digital measurements are repeatable. If an assessment is repeated under the same conditions, the system will record the same movement, pressure and force data every time. It is then the clinician’s role to interpret what those measurements mean for the individual patient.
However, neither approach is meaningful without the expertise to interpret the findings. A clinical gait assessment is not simply about collecting information; it is about understanding what those findings mean for that individual and how they should influence diagnosis and treatment.
Digital analysis does not replace clinical expertise, it enhances it. The technology provides objective data, but it is the clinician’s knowledge, experience and clinical reasoning that transform those measurements into meaningful advice and an effective treatment plan.
Looking at movement is only part of the story
Although watching someone walk provides valuable information, it rarely tells the whole story.
In reality, two people can appear to walk almost identically yet have completely different underlying problems. For example, both may appear to pronate heavily (where the foot rolls inwards), but one may have restricted ankle mobility while the other has weakness around the hip affecting the entire lower limb. Although the movement pattern looks similar, the underlying cause (and therefore the most appropriate treatment) may be very different.
This is why gait assessment is only one component of a wider clinical examination. Understanding why someone moves the way they do is often more important than simply observing how they move.
Is that the same as a shoe shop gait check?
Many people have stood or walked on a treadmill while trying on running shoes. These services can be very helpful for selecting footwear that feels comfortable and suits an individual’s running style.
However, their purpose is different from a clinical gait assessment.
Footwear assessments are designed to help identify shoes that are likely to suit your running style and feel comfortable. For many people, that’s exactly what’s needed. A clinical gait assessment, however, asks a different question:
Why are you moving that way in the first place?
While shoe retailers are highly knowledgeable about footwear and helping customers choose appropriate shoes, they are not carrying out a clinical assessment. A podiatrist has undertaken extensive training in anatomy, biomechanics, pathology and lower limb function, allowing them to interpret movement within the context of a patient’s symptoms, medical history and clinical examination.
Rather than focusing solely on how someone looks while walking or running, a podiatrist considers the wider clinical picture, including joint function, muscle strength, biomechanics, neurological function, medical history, and functional testing before recommending treatment.
Both services have their place, but they answer very different questions.
Understanding movement leads to better treatment
Gait assessment is an important part of understanding many foot, ankle and lower limb conditions.
Whether assessment is performed through skilled visual observation, supported by pressure analysis, or enhanced with advanced digital technology, the goal remains the same: to understand not just how you move, but why.
Technology can measure movement with remarkable precision, but it cannot diagnose, explain symptoms, or recommend the most appropriate treatment. Those decisions rely on clinical expertise.
It is the combination of objective measurement, skilled clinical interpretation, and an understanding of the individual patient that allows treatment to be tailored effectively, helping clinicians address the underlying cause of a problem rather than simply treating what can be seen.
