When used alongside a comprehensive rehabilitation programme, shockwave therapy can help accelerate healing in tissues that have struggled to recover on their own.
Why does recovery sometimes stall?
Most injuries heal naturally. However, conditions such as plantar fasciopathy (commonly known as plantar fasciitis) and Achilles tendinopathy don’t always follow a straightforward path.
As these conditions become more persistent, the affected tissue can become less responsive to the body’s normal healing processes. This is one reason why people often reach a plateau despite doing all the right things. They may notice gradual improvement initially, only to find that recovery slows or appears to stop altogether.
Understanding why recovery has stalled is often the key to getting it moving again.
What is Shockwave Therapy?
Extracorporeal Shockwave Therapy (ESWT) uses controlled, high-energy acoustic waves delivered to the injured tissue.
Rather than simply masking pain, the mechanical stimulus created by shockwave encourages a biological healing response within chronically overloaded tendon and fascial tissue. It promotes tissue regeneration, improves local blood flow and helps “reawaken” the body’s natural healing processes.
Although the name can sound intimidating, the treatment is completely non-invasive and usually takes only a few minutes.
NICE guidance and a growing body of clinical evidence support the use of shockwave therapy for selected chronic tendon conditions when conservative treatment alone has not achieved the desired outcome.
It’s important to remember that shockwave therapy isn’t a quick fix, and it isn’t appropriate for everyone. However, for carefully selected patients, it can significantly accelerate recovery in conditions that are well known for their slow healing, such as plantar fasciopathy and certain chronic tendinopathies.
Shockwave Therapy for plantar fasciopathy
Plantar fasciopathy is one of the most common causes of persistent heel pain.
Typical symptoms include:
- Sharp pain beneath the heel when taking the first few steps in the morning.
- Pain after sitting for prolonged periods.
- Discomfort during long walks.
- Pain after exercise rather than during it.
Many people improve with the right combination of footwear advice, strengthening exercises, activity modification and, where appropriate, orthotics (custom-made insoles).
However, when symptoms have persisted for several months despite an appropriate rehabilitation programme, shockwave therapy can help stimulate further healing.
Most people notice gradual improvement over the weeks following treatment as the tissue responds and recovery progresses.
Shockwave Therapy for Achilles tendinopathy
The Achilles tendon is the strongest tendon in the body, yet it is commonly overloaded through walking, running and sport.
Symptoms often include:
- Pain when taking the first few steps in the morning.
- Stiffness after periods of rest.
- Pain during or after activity.
- Thickening or swelling of the tendon.
Achilles tendinopathy responds particularly well to a carefully progressed strengthening programme. However, when progress plateaus despite appropriate rehabilitation, shockwave therapy can be an effective addition to help restart the healing process.

At Feet & Motion, we don’t see shockwave therapy as a standalone solution.
In fact, many people with plantar fasciopathy or Achilles tendinopathy recover without ever needing shockwave therapy. We only recommend it when we genuinely believe it’s likely to improve your outcome.
Every treatment begins with a comprehensive assessment to understand not only what is hurting, but why.
